What if the hospital will not provide financial assistance materials?
Patient Bill Community Team educational note: This discussion was prepared for general education by the Patient Bill Community Team. It is not a real patient story and is not individualized medical, legal, or financial advice.
A request ladder, not a debate
When financial-assistance materials are hard to locate, use a short sequence of specific requests. For a hospital facility covered by section 501(r), the financial assistance policy, application form, and plain-language summary must be widely publicized; paper copies must be available without charge in public locations and on request. The regulation is available at 26 CFR § 1.501(r)-4.
Step one — ask for the full set. Request the policy, application, and plain-language summary together. Asking for only “charity care information” can produce an incomplete answer.
Step two — name the format. If online access is difficult, request a free paper copy or ask what accessible format is available.
Step three — redirect precisely. If the first department cannot help, ask for the office, mailing address, portal path, or phone menu responsible for the facility’s materials. A patient financial counselor or compliance office may be able to identify the correct route.
Sample wording: “I am requesting free copies of the current financial assistance policy, application, and plain-language summary for this facility. If this office does not provide them, please identify the office and official method for obtaining them.”
What to preserve
Make a small contact note with the department, general method used, and whether materials were sent. Save the version you receive because online pages can change. Do not rely on a search-result snippet or a policy from another campus without confirming that it applies to the biller named on the statement.
This is an information-gathering process, not a conclusion about eligibility. A separate professional bill may follow another organization’s rules. If a response is vague, ask a narrower follow-up: “Does this document apply to the entity named on my statement?”
