NY, California, and Texas: pick a starting portal

Three-state portal field guide
Start with geography, then verify product and agency scope. A portal can accept information without necessarily being the authority that decides the issue.
| Location | Official doorway | Question before filing |
|---|---|---|
| New York | Department of Financial Services | Does DFS handle this coverage and issue? |
| California | Department of Managed Health Care or Department of Insurance | Which department regulates this health product? |
| Texas | Department of Insurance | Is the coverage within TDI’s complaint scope? |
Pack a private one-page brief
Use five headings: coverage type, decision being challenged, steps already taken, result requested, and next deadline shown on the notice. Under each heading, use a short factual sentence. Keep the full notices available for a secure submission rather than copying sensitive details into an initial public question.
Confirm before upload
Check that the site uses the agency’s official government domain, read its privacy and attachment instructions, and note whether it requests completion of an internal appeal first. If the agency does not oversee the product, ask for the correct official referral. Never send records to an address supplied only in a forum reply.
California has two entries intentionally: DMHC and CDI have different regulatory roles, so residence alone does not select between them. New York DFS and Texas TDI are likewise starting points, not automatic rulings that a charge is covered or that a complaint belongs there. Self-funded employer plans and federal programs can require other channels.
Maintain the appeal described by the insurer or program notice while checking jurisdiction. Filing an agency inquiry should not be assumed to extend that separate deadline. Save the portal receipt, attachment list, and agency response in the private case folder.
