What belongs in a records request before questioning a bill?
Editorial note: Prepared by the Patient Bill Community Team for general education. This is not a real patient story and is not individualized medical or legal advice.
Document map
Use this prompt to sort paperwork before requesting anything. Match each question to an owner:
| Question | Useful document | Likely owner |
|---|---|---|
| What was charged? | Itemized statement | Provider billing |
| How was a claim processed? | EOB or claim detail | Health plan |
| What record explains a service? | Limited relevant record | Provider records |
Mini worksheet
Write one unexplained line in general terms, then complete: “I need ___ because I am trying to understand ___.” That phrasing prevents a broad request for every record. A community reply can suggest categories of documents, not interpret an individual record.
Privacy boundary
Do not post images, names, dates, addresses, account numbers, or medical details. Use the provider’s records process for access questions and the plan’s secure channel for coverage records. A useful reply to this prompt can explain why a bill, EOB, and clinical record serve different purposes. It should not ask another member to reveal a line item. Before sending a request, remove unrelated pages and state whether the goal is explanation, correction, or access. Choose the smallest document that can answer the question: an itemized statement may resolve a billing question without a broad clinical-record request. CMS consumer guidance is a starting point; the organization’s instructions control the request.
