External-review question matrix

AskWhy it matters
Which final decision is being reviewed?External review is decision-specific.
What deadline and form apply?Process requirements vary.
Is expedited handling available?Urgency has defined conditions.
Is the request complete?Confirmation prevents avoidable gaps.

This is a document-reading exercise, not a prediction that external review will be available or successful. Provider billing errors may require provider review instead. Keep the final plan decision private and use the plan or official channel for case-specific questions.

HealthCare.gov’s external review page explains the general federal framework. For a discussion exercise, sort questions into eligibility, timing, materials, and confirmation. That structure is different from arguing the merits of a denial and helps prevent a missing-form question from being overlooked. Do not paste a final decision or case reference into the thread; the matrix works without it.

Read the final decision as a route map

Privately mark the paragraph that states whether internal review is complete, the entity receiving an external request, the filing period, and any expedited procedure. Then ask a process-only question: “Does this final decision qualify for your external-review process, and which form and supporting items are required?” Request a written answer if the notice and website point to different administrators.

Build a separate folder for the final denial, external-review form, evidence actually submitted, representative authorization if required, and receipt. Do not mix earlier draft arguments into the filed set unless they support the specific disputed determination. If the administrator says the issue is outside external review, ask for the reason and any named alternative complaint route. That is the escalation boundary: eligibility staff identify the process; they do not resolve provider bookkeeping or provide personal legal interpretation.
