Good faith estimate threshold and timing check

Calculation card for self-pay paperwork

This exercise concerns a good faith estimate and the initial bill for the relevant care. Work privately from itemized documents.

Step 1: align the rows

Group documents by the provider or facility that issued them. Place each entity’s estimate beside its initial bill and mark every charge for the relevant care, including associated services missing from the estimate. Keep unrelated care and separately billing entities in distinct comparisons.

Step 2: do the arithmetic

provider or facility’s total billed charges for the relevant care
minus that provider or facility’s total good faith estimate
equals difference to check against the threshold

Do not exclude an associated charge merely because it was unlisted. An added or changed service does not automatically disqualify a dispute; whether an unforeseen service justifies additional payment is a separate determination.

Step 3: ask for reconciliation

Send: “Please reconcile this good faith estimate with your total billed charges, identify additions or changes, and itemize the difference.” Retain the response and original documents.

Step 4: check the current federal criteria

CMS describes a federal dispute process that may be available for certain uninsured or self-pay bills when charges are at least $400 above the good faith estimate. The direct CMS dispute instructions require an initial bill dated within the preceding 120 calendar days when submitting a dispute. This is a screening checkpoint, not an eligibility conclusion.

Calculate the filing window from the date printed on the initial bill—not when it arrived and not a later statement. Requesting clarification from billing does not suspend that window.

Never post estimates or itemized statements; they can reveal sensitive care and identifying data. Verify current forms, fees, and eligibility details on the CMS instructions before taking a formal step.
